Participation Agreement & Acknowledgment

By submitting this application, I, the undersigned, wish to take full contact martial arts instruction, classes, and training, and to participate in activities, events, and competitions that include but are not limited to Karate, Kickboxing, Boxing, Grappling Arts, and MMA at the American Institute of Kenpo (AIK). I am willingly choosing to participate in full contact martial activities, including events and competitions that may or may not be sponsored in part or in full by AIK, its members, or affiliates.

I know, recognize, acknowledge, and accept that full contact martial instruction, classes, training, activities, events, and competitions can be a demanding physical activity and that participation has certain inherent risks. Those risks may include serious illness, injury, and death. I also know, recognize, acknowledge, and accept that prior injuries can increase the risk of serious illness, injury, and death. I hereby knowingly and voluntarily assume those risks as a condition of being allowed to participate in training at AIK. I hold harmless AIK, its members, employees, and agents for any and all related full contact martial instruction, classes, training, activities, events, and competitions.

I hereby waive and release in advance any claim that I may hereafter acquire against AIK, its members, employees, and agents, arising out of any physical, mental, or emotional illness or injury that I may sustain while participating in instruction, classes, training, activities, events, and competitions. I understand, intend, and will waive and release these claims even if they arise wholly or in part out of the negligence or intentional actions of the members, employees, or agents of AIK.

Public Image Consent: I consent to any pictures, video, or other likeness taken of me in connection with AIK, its members, employees, or agents for publicity, promotion, website, social media (including Facebook and Instagram), television, or other public use. I understand that other students or visitors may also capture images or video during classes or events, and that AIK is not responsible for images shared by third parties. I hereby waive any claim for compensation or liability arising out of such use.

I (and/or the Parent/Guardian) affirm that the applicant is physically fit and has medical clearance to engage in this type of training.

This agreement is effective for one year from the date of submission. The applicant (and parent/guardian, if applicable) must review and re-acknowledge this agreement annually, beginning one year from the original submission date, or upon any material change to the agreement. Continued participation after the renewal date without a new acknowledgment may result in suspension from the program until the agreement is renewed.

If the applicant is under 18, this agreement must be acknowledged by a parent or guardian.
